- Postal address where the equipment will be delivered in the future
- IMEI number of the telecom equipment
- If there is more than one telecom equipment, then the total quantity must be mentioned with the IMEI, brand name and model number of each equipment.
As the authority of providing the NOC is with the Pakistan telecommunication authority.
